EVALUATION FORM

Please bring the following when coming to the pool:

● Swim Suit, Towel, Swim Cap (girls), Racing goggles (Competitive Swimming Goggles are best)

● A smile and excitement about swimming.

Provide swimmer Information. If you have multiple swimmers, please complete one form for each swimmer.

Pre-requisites: Able to swim one length of unassisted Freestyle.

RELEASE OF LIABILITY WAIVER

By checking the box below, I, the undersigned participant or legal guardian of a participant, intending to be legally bound, do hereby for ourselves, our heirs, executors and administrators, hold harmless, waive and release and forever discharge any and all rights and claims for damages or injury which we or either of us may hereafter have against South Orange County Aquatics, and its employees, agents, officers, coaches, representatives, and volunteers.

Your interest form has been received! A member of our club will reach out shortly to schedule an evaluation. Once the evaluation on the pool deck has been completed, you will be contacted via email regarding your swimmers acceptance onto the team.
